「as follows」は、「以下のとおり、次のとおり」という意味のフレーズです。
“as it follows =（後ろに）続くように” という元々の構造から考えれば、意味はストレートですね。
接続詞 as の後ろに主語は入れず、「as follows」の形でいつも使います。
定型のため、主語の単複を気にする必要はありません（ follows の三単現の s が付いたり取れたりと変形することはない）。
